Our athletic training staff at LEE High School are highly certified professionals that are trained in preventing, recognizing, managing, and rehabilitating injuries that are a result of physical activity. Here, you will find pertinent information regarding athletic physicals, online participating forms, and other information pertaining to your student's participation in sports. Athletic Physical
Athletic physicals are required prior to students participating in any sports here at LEE High School. This form consists of two parts: a medical history questionnaire and a physical examination. Click on the current form below. For any questions regarding the athletic physical, please contact a coach or the Athletic Trainers on campus.

2024-2025 Athletic Participation forms should be completed through Rank One Sport by clicking the link below. If you have any questions or need a printed copy of the Athletic Participation form, please contact the Athletic Trainers on campus. *Please note that there are two participation forms available; high school and middle school.
